Yampplink --> FT245BM ----> ATmega162 connection failed

hi all, when i was trying to upload firmware through yampplink, the led blink after enabling bootloader, but by the time i tried to upload firmware, the led blink at a slower rate and the yampplink freeze a little while and display "time out error". I have tried all the method mentioned in the forum before even the spi exerciser. Any help are highly appreciated, THANKS IN ADVANCE.  :'(

my setting is as below:

fuse setting:

- CKSEL0 fuse PROGRAMMED,
- CKSEL2 fuse PROGRAMMED,
- CKSEL3 fuse PROGRAMMED,
- SUT0 fuse PROGRAMMED,
- CKDIV8 fuse PROGRAMMED,
- BOOTRST fuse PROGRAMMED,
- SPIEN fuse PROGRAMMED,

Crystal:
-------------------
using ATmega162 internal RC at 8.00MHZ
tried 4MHZ, 6MHZ, 13.50MHZ but doesn't help (with correct fuse setting to disable internal RC )

PC setting:
--------------------
using FTDI VCP driver v2.00 on winXP
driver VCP port = com9

yampplink port = com9

As always, check for solder errors and correct component mounting.
Then check it again.
And once more to be sure.

i have rechecked almost 10 times by multimeter, it couldn't be any pcb error. by the way why does the bootloader LED blinking at slower rate when the RXF pin on the ATmega162 detects a signal from the FT245BM TXF pin ? what does it means ? i can't find it in the bootloader.asm or it is beyond my knowledge, Jesper please help
